Nursing Home Transition

 Nursing Home Transition programs throughout the commonwealth provide alternatives to nursing home care to individuals age 18 and older. CILNCP provides this service to individuals age 18 to 59 in Centre, Clinton, Columbia, Lycoming, Montour, Northumberland, Snyder, and Union Counties.

Transition Coordinators are available to provide outreach and service coordination to all area nursing facilities and residents, with the goal of relocation from institutions to community living.
