Aging Waiver

Program Description: The Aging 60+ Waiver provides Home and Community Based services to eligible persons age 60 or older who are clinically eligible for nursing facility care.

Target Population: Individuals who are age 60 or older who are nursing facility eligible.

Eligibility Criteria:

  • Individuals age 60 or older
  • Income limit must be equal or less than 300 percent of the Federal Benefit Rate
  • Individuals must have a level of care for a Skilled Nursing Facility
  • Resources are $2,000 or less
