Specialized Services through Liberty Resources as a fee for service through OLTL.

  • Community Integration-based on the resident’s Specialized service plan, the Specialized Service Plan Coordinator arranges for community integration such as shopping or going to the mall for the first time. These services increase the consumer’s level of independence and provides him/her with socialization skills at the same time
  •  Transportation- the Specialized Service Plan Coordinator assists nursing home residents with coordinating transportation to attend community integration activities;
  • Support Coordination and Advocacy-the Specialized Service Plan Coordinator meets with the nursing home resident  and  the nursing home representative to develop a Specialized service plan, maintain the services in the plan, advocates for the resident on issues regarding the services provided in the nursing home;
  • Peer Counseling- Specialized Service Plan Coordinators arrange for persons who have disabilities and live outside the nursing home who are able to act as a role model or mentor for residents. Peer Counseling is also provided by SUBCONTRACTOR;
  • Independent Living Skills Training- The training is based on the resident’s need, either one-on-one or in a group. Training is provided for Independent Living, cooking, transportation, managing your attendants, budgeting, etc;
  • Equipment Assessment- Specialized Service Plan Coordinators work with the resident to identify necessary equipments needed to transition from the nursing home to the community. Resources including Medical Assistance must be exhausted before OLTL pays for the equipment;
  • Relocation/Transitioning – The Specialized Service Plan Coordinator works with the resident to identify community living alternatives.  The SSP includes all of the services listed above to assist the consumer to develop necessary skills for successful independent living.  The Coordinator will assist the consumer to identify and facilitate meetings of the consumer’s Community Support Team;
  • Assistance with Moving – staff assists the consumer with the actual transition including establishment of his/her new household, shopping, utilities and finances (transfer of subsidies).  The affiliated agencies under this project will ensure that the consumer’s health and safety needs are met;
  • Ensuring waiver enrollment and or referrals to other needed community services
  • Back up Plans – the Specialized Service Plan Coordinator will work with the consumer to ensure that he or she has additional personal assistance plans if primary caregivers are unavailable;
