Community Integration

MINIMUM QUALIFICATIONS

​

  • Must be at least 18 years of age.

  • Must have at least a high school diploma or GED.

  • Must be able to perform all duties described within the individual consumer(s) Individualized Service Plan.

  • Must possess a valid driver’s license, vehicle, and valid car insurance for the duration of employment.

  • Must have flexible availability to perform scheduled work, including possible evenings, weekends, and/or holidays.

  • Must submit to criminal and federal history reports and background checks, and not be prohibited from hiring or continued employment with the agency.

 

ESSENTIAL RESPONSIBILITIES

​

To assist, help, encourage, or support our consumers to develop and sustain a range of valued social roles and relationships, build natural supports, increase independence and potential for employment, and experience meaningful community participation and inclusion.  This can include but is not limited to providing the service as follows:

​

  • Participating in community activities, organizations, groups, or clubs to develop social networks.

  • Identifying and participating in activities that provide purpose and responsibility.

  • Fine and gross motor development and mobility.

  • Participating in community opportunities related to the development of hobbies or leisure/cultural interests or promoting personal health and wellness.

  • Participating in community adult learning opportunities.

  • Participating in volunteer opportunities.

  • Participating in opportunities focused on training and education for self-determination and self-advocacy.

  • Learning to navigate the community, including public/private transportation (bus/Uber/Lyft/MA transportation).

  • Developing and/or maintaining social networks and reciprocal relationships with members of the broader community (neighbors or coworkers).

  • Identifying and utilizing support from community service organizations such as churches, schools, libraries, clubs, businesses, and community organizations focused on the exchange of services.

  • Developing skills and competencies necessary to pursue competitive integrated employment.

  • Assist with individualized plan development.
